- In order to proceed with your scheduled appointment at Salon Blo, the requirements noted below must be adhered to
- Once you have reviewed the items below, you are requested to confirm your appointment by the confirmation text you receive in advance.
COVID -19 SCREENING REQUIREMENTS FOR SALONBLO STAFF AND CUSTOMERS
1. Have you been tested for COVID-19 and awaiting test results?
2. Have you travelled outside of Canada in the past 14 days?
3. Have you tested positive for COVID-19 ?
4. Have you had close contact with a confirmed case of COVID-19 without wearing appropriate PPE?
5. Do you have any of the following symptoms?

● New onset of cough

● Worsening chronic cough

● Shortness of breath

● Difficulty breathing

● Sore throat
-
● Hoarse voice

● Difficulty swallowing
-
● Decrease or loss of sense of taste or smell

● Chills

● Headaches

● Unexplained fatigue/muscle aches

● Nausea/Vomiting, diarrhea, abdominal pain

● Pink Eye (conjunctivitis)

● Runny nose or nasal congestion without other known cause
