Appointment Confirmation Protocol
 
  1. In order to proceed with your scheduled appointment at Salon Blo, the requirements noted below must be adhered to
  2. Once you have reviewed the items below, you are requested to confirm your appointment by the confirmation text you receive in advance.

 

COVID -19 SCREENING REQUIREMENTS FOR SALONBLO STAFF AND CUSTOMERS

1. Have you been tested for COVID-19 and awaiting test results?
2. Have you travelled outside of Canada in the past 14 days?
3. Have you tested positive for COVID-19 ?

4. Have you had close contact with a confirmed case of COVID-19 without wearing appropriate PPE?

5. Do you have any of the following symptoms?

  • ●  Fever

  • ●  New onset of cough

  • ●  Worsening chronic cough

  • ●  Shortness of breath

  • ●  Difficulty breathing

  • ●  Sore throat

  • ●  Hoarse voice

  • ●  Difficulty swallowing

  • ●  Decrease or loss of sense of taste or smell

  • ●  Chills

  • ●  Headaches

  • ●  Unexplained fatigue/muscle aches

  • ●  Nausea/Vomiting, diarrhea, abdominal pain

  • ●  Pink Eye (conjunctivitis)

  • ●  Runny nose or nasal congestion without other known cause

  •  

  • If you answer YES to any of these questions

  • we will be required to reschedule your appointment.

  • Please call to make the necessary arrangements.

My appointment is at 5:30 pm on Friday June 26th, not 4:15. My daughter Elyse Henri's appointment is at 4:15, before mine.
